The support we provide is based on a assessment of the individual’s needs. The Registered Manager/Care Co-Ordinator will assess and discuss your requirements, listening to your needs. They will explain the service and the information pack, discuss consent and care plans. The service will be tailor-made to your requirements whilst respecting your wishes throughout, developing the care plan together. We will seek to match you with one or more Support Workers and pride ourselves in ensuring continuity of care by providing the same support workers. This creates stronger relationships with the person, carer and families and assists in giving peace of mind.
